"This document describes a process for obtaining consent to immunization, and establishes a standard of practice for obtaining consent for a vaccine or vaccine series. Sections 2 to 6 address consent for immunization of infants, children and capable adults. Section 7 addresses consent for immunization of adults incapable of consenting on their own behalf. This standard is limited to describing the process of obtaining consent for immunization. Although an assessment of the individual’s health is an essential component of the decision to immunize, it is not part of the informed consent process

  • Guidelines for assessing authority and capability
  • The Standard Information to be provided
  • Guidelines to assess understanding
  • Documentation of consent or refusal
